0186(1) Licensor observed an enrolled child's file with a food allergy did not obtain a written instruction ICP from the child's health care provider and parent with a known food allergy or special dietary requirement due to a health condition. 0186(2) Licensor did not observe the necessary medication, training, and equipment to properly manage a child's food allergy.
